چکیده

A review of the literature suggests that a significant number of patients undergoing anterior cruciate ligament (ACL) reconstruction continue to suffer residual pain and instability after surgery. Consequently, there remains considerable room for improvement in surgical ACL reconstruction. Anatomically, the ACL can be divided into 2 major functional bundles, the anteromedial and the posterolateral bundle. Biomechanical studies have shown that both bundles play important roles in providing anterior and rotational stability of the knee. In particular, it has been demonstrated that biomechanically, anatomic double-bundle ACL reconstruction results in a better restoration of the rotational stability of the knee joint. We suggest that anatomic ACL double-bundle reconstruction may result in a better restoration of the in vivo kinematics of the knee joint and can improve the clinical outcomes in ACL surgery. Currently, our preferred surgical technique uses 2 separate tibial and femoral tunnels using 2 tibialis anterior or tibialis posterior tendon allografts. Future clinical studies are necessary to evaluate the long-term functional outcomes after anatomic ACL doublebundle reconstruction.

Biomechanical comparison of single-tunnel-double-bundle and single-bundle anterior cruciate ligament reconstructions.

BACKGROUND Anatomic double-bundle reconstruction has been thought to better simulate the anterior cruciate ligament anatomy. It is, however, a technically challenging procedure, associated with longer operation time and higher cost. HYPOTHESIS Double-bundle anterior cruciate ligament reconstruction using a single femoral and tibial tunnel can closely reproduce intact knee kinematics. STUDY ...

Anatomical Individualized ACL Reconstruction

The anterior cruciate ligament (ACL) is composed of two bundles, which work together to provide both antero-posterior and rotatory stability of the knee. Understanding the anatomy and function of the ACL plays a key role in management of patients with ACL injury. Anatomic ACL reconstruction aims to restore the function of the native ACL. Femoral and tibial tunnels should be placed in their an...

Biomechanical comparisons of knee stability after anterior cruciate ligament reconstruction between 2 clinically available transtibial procedures: anatomic double bundle versus single bundle.

BACKGROUND Several trials have compared the clinical results between anatomic double-bundle and single-bundle anterior cruciate ligament reconstruction procedures. However, it remains controversial whether the anatomic double-bundle procedure is superior to the single-bundle procedure. HYPOTHESIS The anatomic double-bundle procedure will be better than the single-bundle procedure at resisting...

Restoring Nature Through Individualized Anatomic Anterior Cruciate Ligament Reconstruction Surgery

Anterior cruciate ligament (ACL) reconstruction surgery has significantly evolved in recent years. This has led to development of new technologies that facilitate the diagnosis of ACL injury and the application of state of the art methods for treatment. In particular, individualized anatomical ACL reconstruction aims to restore native ACL function. Treatment is tailored to each patient based on...

Current Trends in Anterior Cruciate Ligament Reconstruction

The advances in the knowledge of anatomy, surgical techniques, and fixation devices have led to the improvement of anterior cruciate ligament (ACL) reconstruction over the past 10 years. Nowadays, double bundle and anatomical single bundle ACL reconstruction that more closely restores the normal anatomy of the ACL are becoming popular. Although there is still no definite conclusion whether doub...

Anatomic Double-bundle ACL Reconstruction.

Rupture of the anterior cruciate ligament (ACL) is one of the most frequent forms of knee trauma. The traditional surgical treatment for ACL rupture is single-bundle reconstruction. However, during the past few years there has been a shift in interest toward double-bundle reconstruction to closely restore the native ACL anatomy. This paper evaluates the basis for double-bundle ACL reconstructio...
